Dr. Hogan Insurance & Payment

I am in-network with traditional Medicare and will soon be in-network with BCBS, BCBS Medicare Advantage, Tufts Medicare Advantage. For private pay, I will provide you with a super bill to submit to your insurance for out-of-network reimbursement. Therapy sessions are $225 and adult ADHD assessments are $1500. I accept major credit cards and require all clients to keep a credit card on file in my secure and HIPAA compliant portal.

Cancellation Policy

Clients will be billed for the full fee session rate with less than 24-hours or no notice (no show).

About Out-of-Network Insurance for Private Pay

For private pay clients who want to submit superbills from me to your insurance provider for out-of-network reimbursement, you will need to contact your insurance provider for information about your benefits. Below is guidance for how to communicate with them to determine your out-of-network reimbursement benefits:

• Call the member services number on the back of your insurance card.

• Ask if your plan offers out-of-network benefits for mental health services.

• Ask if you have a deductible to meet before out-of-network mental health services are covered?

• If yes, how much is the deductible? How much has been met already?

• Once the deductible is satisfied, what percentage of the psychologist’s bill do you reimburse for out-of-network mental health services?

• Are there any limitations to this coverage (e.g., session limits)?

• Ask how to file claims for reimbursement.

• Write down the representative's name and a reference number for the call. This information will be necessary if you ever need to dispute a rejected claim.
